  • For manual processing, a letter request
  • Sheet for Monitoring
  • OFWs must have a certificate of coverage for required insurance.

  • Passport photocopy
  • For domestic workers, low-skilled female workers with a foreign placement agency as an employer, drivers bound to the Middle East, and overseas performing artists for Korea and Japan, Taiwan only, and individually validated employment contract
  • OWWA‘s Comprehensive Pre-Departure Education Program (CPDEP) Certificate, National Certificate (NCII),  and PDOS Certificate (for HSWS)
  • Comprehensive motor vehicle insurance with third-party liability is recommended for drivers traveling to the Middle East.
  • For female workers, a copy of a visa or an equivalent document (with an English translation) is required.
  • Signatory card to the agency

For Processing via e-Submission:

  • An accomplished E -Submit Form
  • Sheet for Monitoring
  • OFWs must have a certificate of coverage for the required insurance.
  • Contract of employment with individual verification (for domestic workers, low/semi-skilled female workers with a foreign placement agency as an employer, drivers going for the Middle East, and only Korean and Japanese performing artists)
  • OWWA CPDEP Certificate, PDOS Certificate, National Certificate (NCII) (for HSWS)
  • Comprehensive motor vehicle insurance with third-party liability is recommended for drivers traveling to the Middle East
  • For female workers, a copy of a visa or an equivalent document (with an English translation) is required.

  • Processing fee of 200 pesos(4$) per worker
  • US$25.00 or Peso equivalent OWWA Membership Fee (per worker)
